Do we still not know how apprentice xehanort "died", and therefore produced his heartless Ansem SoD and his nobody Xemnas? Or was this explained somewhere?

Yeah, it was explained in the Ansem Reports and Secret Ansem Reports in Kingdom Hearts I and Kingdom Hearts II.
