There are two types of Heartless, Pure blood - which occured naturally when people fell to darkness, and emblem, the Heartless with the mark on them. The Heartless Machine created the Emblem Heartless as the result of Xehanort's experiments to see if he could replicate the process.
I know but do you know that in hollow bastion heartless came out of the final keyhole right?
And that has to do with the facts Tinny mentioned exactly how?
The one doesn't rule out the other.
The final Keyhole wasn't even complete before Sora sacrificed himself to free Kairi's heart.
Heartless could come and go between worlds as they pleased because Xehanort had opened the door to the Heart of Radiant Garden and brought down all protective walls between the worlds as well as the Realm of Darkness and the Realm of Light.
The final Keyhole was just a means to an end for Xehanort to speed up the process and allow the Heartless to swarm the Realm of Light in even greater numbers.

When AX opened the door to the heart of Radiant Garden, it didn't break all the world's barriers. It just broke RG's. Every world has their own barrier, and when you open the heart to that world the barrier breaks. The heartless were already spreading by the time King Mickey came to speak to Ansem the Wise because he used a gummi ship...which is made out of the world's barriers
