I got mine against Xigbar. I actually didn't know I got it, later when fighting Saix instead of going Master (I don't look at my drives, I select the last command for master, first for valor etc) I went into something silvery, I was like "oh cool" then he went "moon shine down" then while he was berserk my final ran out.

You can get Final Form in KHII.

You can only get the Drive Form that resembles your old outfit somewhat in FM+, though.
