Nomura confirmed in an interview that only Xehanort's heartless took a form like that. All the rest became normal heartless, like we fought in the various worlds in KH1. So maybe we killed them already =D

Precisely. Even in Ansem's Secret Reports, he theorized that Xehanort's heartless was the only one to do so. The others he assumed they just became regular heartless.
