Spoiler ShowJoshua in the secret ending might mean:
1) He is still unpleased with Shibuya, and although he did not proceed to erase Shibuya, he doesn't feel happy about it. Such feelings only grows.
2) Something else in Shibuya had bothered Joshua, he was thinking whether he should bother his 'Player', Neku preferably since he's the best, by sending him into another 'Game' to eliminate the threat. But since he has developed a liking to these children, he was doubtful.
3) He was bored? It seemed that way.
I hope that there would be a sequel. I suspect that if there is one, there would be a new character and we most likely won't be using Neku.
However, in order to explain the anomaly in that sequel, they would create another side game which describes Neku's 4th Week (Which we never get to play), and it would most likely tell most of the story.
