"The Heartless Prophecy" as it is not officially called, was in the End of the World in Kingdom Hearts 1, you may not remember because you could only see it once and then never again

Right around when you fight Chernabog you get transported to a place resembling Hollow Bastion, the place appears to be a laboratory of some sort and when you examine the computer it reads you that poem, basically out-lining the Heartless and how the Door to Kingdom Hearts must be sealed

I'm sure plenty of you can figure out what it's supposed to mean..but then again I'm sure many of you won't...so I'll sum it up for you

This is describing the Heartless. They are born from Darkness, more specifically the Darkness within a person's Heart. They seek the heart of the world, and then they consume it, destroying that world

The final line is rather enigmatic, I think basically it's explaining that their one goal is to destroy all hearts until no heart remains but the Great Heart (Kingdom Hearts). When a heart is taken, it returns to Kingdom hearts, so their ultimate goal basically is to make Kingdom Hearts take all the Hearts

This describes Kingdom Hearts, within the "Great Heart' of Kingdom hearts there is a great darkness

The second line means that all Heart eventually return to Kingdom hearts, so all Hearts eventually return to Darkness

Seven hearts, one Keyhole, one key to the door.

A Heart of Light shall reveal unseal the path to Kingdom hearts, this means Sora

The Seven Hearts refers to the Princess' of heart. The Princess' gather and unseal the keyhole that opens to path to Kingdom hearts (The keyhole in Hollow Bastion, where we fought Xehanort within Riku in KH1), and the Keyblade unlocks that door

When that keyhole is awakened and eventually unlocked, then the path to Kingdom Hearts shall be revealed

The door of darkness, tied by two keys.
The door of darkness to seal the light.

The Door to Kingdom Hearts, on one side of the Door is the Key to Light, on one side is the Key to Darkness, only those 2 keys can seal the Door

The Door has the power to seal away Light

None shall pass but shadows, returning to the darkness.
Ones born of the heart and darkness, hunger for every heart until the dark door opens.

No one can pass through the Door but beings of Darkness, so that means the heartless and those who's heart contain darkness

The Heartless, who are born of the Heart and Darkness, will forever hunger for Hearts, they will consume as many hearts and as many worlds until the Dark Door is finally opened, and when that happens to sort of event will happen
