So I haven't seen a lot of information concerning the stats and abilities I've wanted to over Kingdom Hearts III's Keyblades and Formchanges, so I've compiled some info from other sites and a LOT of info from my own play-through. If anybody's interested, these are accurate descriptions of abilities and any changes in combat that take place (In the Descriptions, I avoided stating the same thing if they were mentioned in Abilities of Form Actions).

Kingdom Key:
Your very first Keyblade.

Strength / Magic: LV0 - 4/3 -> LV10 - 9/8
Abilities: 1. Treasure Magnet = Allows user to attract dropped prizes and items from a greater distance away. Powering up the ability increases the range of the effect / 2. Formchange Extender = Extend the availability period of formchange commands. Stack the ability to increase the effect.
Shotlocks: King of Hearts: Fire a burst of magical projectiles from the end of your Keyblade -> (MAX) Ragnarock: Fire streams of brilliant light that fly at targets.

Abilities: 1. Wizard’s Ruse = Potentially recover HP proportionate to the MP you expend. Stack the ability to increase the odds / 2. Chef Extraordinaire = Earn an Excellent rating more easily when cooking with Little Chef.
Shotlocks: Steam Spiral: Spin your Keyblade and send fireworks careening towards targets -> (MAX) Fruit Crusher: Use a shield to scatter fruit in all directions.

Formchange: Frying Pan (Blitz Form)
Obtained: Get Bistro to 5-Star rating by getting Excellent in all cooking recipes.

Ultima Weapon: The supreme Keyblade.
Strength / Magic: 13/13
Abilities: 1. Combo Boost = Increases ground combo damage in proportion to the length of the combos. Stack the ability to increase the effect / 2. Air Combo Boost = Increases aerial combo damage in proportion to the length of the combos. Stack the ability to increase the effect / 3. Situation Boost = Fill situation command gauges more quickly as you defeat enemies.

Second Form (S, D, M, P)

Keyblade(s): Kingdom Key / Dusk Till Dawn / Midnight Blue / Phantom Green / Starlight
Description: If Sora is wielding the Dusk Till Dawn, the Formchange is titled “Second Form D”; if Sora is wielding the Midnight Blue Keyblade, the Formchange is titled “Second Form M”; if Sora is wielding the Phantom Green Keyblade, the Formchange is titled “Second Form P”; if Sora is wielding the Starlight Keyblade, the Formchange is titled "Second Form S". The colour scheme of his clothes change to that of his Kingdom Hearts II outfit. While his Keyblade does not transform, Sora's combat changes heavily. He is able to use old abilities that were present in both Kingdom Hearts and Kingdom Hearts II. Sora is able to perform Limit-like Finisher Situation Command attacks. Sora's Finish command attacks are all attacks taken from the original Kingdom Hearts/Kingdom Hearts Final Mix. Unlike other Formchanges that Sora possesses, the Finish Situation Command is readily available upon engaging Second Form, but using any of the commands end the form prematurely. Every time the Command meter is filled, the Situation Command changes into another, more powerful Finisher.

Ripple Drive: Generate a magical aura that engulfs surrounding enemies. Your Magic stat determines damage. (Combo finisher: while grounded)
Hurricane Winder: Somersault at enemies and hit with a devastating series of blows (Combo finisher: while airborne)
Aerial Finish: Issue a devastating flurry of attacks that sends enemies reeling. (Combo finisher: while airborne)
Magnet Burst: Draws foes in, then expel them with a devastating blast. Your Magic stat determines damage. (Combo finisher: while airborne)
Second Form (D, M, P)
Finisher(s): Stun Impact: Project a stick wave that stuns enemies. Your Magic stat determines damage. -> Sonic Blade: Strike at the enemy as you race by. Continue pressing [EX] when prompted to perform additional attacks. -> Ars Arcanum: Ravage the enemy with a devastating flurry of attacks. Continue pressing [EX] when prompted to perform additional attacks.
Second Form S
Finisher(s): Stun Impact: Project a stick wave that stuns enemies. Your Magic stat determines damage. -> Sonic Blade: Strike at the enemy as you race by. Continue pressing [EX] when prompted to perform additional attacks. -> Ancient Light: Annihilate surrounding foes with five columns of crisscrossing light. Your Magic stat determines damage.

Rage Form
Keyblade(s): N/A
Description: This Formchange is made available if Sora’s HP is reduced to critical levels either while at least one party member is KO’d, or at random when Sora is alone. In this Form, Sora's skin and hair become ink black, his eyes turn yellow with a red glow, and his clothes become black with blue and grey glowing accents and patterns. He leaves behind trails of dark red flames in his wake when he moves. While his Keyblade does not transform, Sora's combat changes heavily.Activating this Formchange restores Sora’s HP to full. Sora is able to unleash rapid, multi-hitting strikes with reckless abandon using the Keyblade, teleporting to and around his target with each attack. However, he is unable to use magic, Items, or Links, and activating any situation commands with party members will end Rage Form prematurely. Finisher: Riskcharge - Readily available upon engaging the Form. When used, Sora sacrifices half of his remaining HP to increase his Strength and he is free to use it multiple times in order to stack the ability, also increasing the time left in Rage Form slightly each time it is used. The visual effect of Riskcharge is a pink wisp following Sora around, representing the pieces of his heart as well as the amount of times the command has been used. The Finish Command attack appears when the maximum of 3 Riskcharges are used, and has Sora attack wildly around himself creating multiple dark spheres, before bursting them and inflicting heavy damage before reverting.

Guardian Form
Keyblade(s): Hero’s Origin / Hunny Spout / Classic Notes
Description: Sora's clothes become yellow with diamond patterns. This Formchange is focused on Sora's defensive capabilities
Abilities
Aerial Recovery: Press [CIRCLE] after taking damage in the air to right yourself.
Barrier: Press [SQUARE] to guard against attacks from all directions

Counter Blast: Press [EX] after a successful block to send surrounding foes flying (Reprisal)
High Jump: Leap high into the air. The longer you hold [CIRCLE], the higher the jump.
Combo Master: Combo continues even if your swings miss the enemy
Second Chance: Always retain at least 1 HP if your are hit while you have 2 HP or more.
Withstand Combo: Always retain at least 1 HP if you are hit while you have 2 HP or more HP. You will remain protected throughout the enemy’s combo.
Hidden Potential: Gain additional Strength and Magic when battling powerful foes.
Defender: Potentially increase Defence by 2 when your HP is at 25% or below. Stack the ability to increase the odds.
Hero’s Origin: Counter Shield
Description: Sora’s Hero’s Origin Keyblade transforms into a Thunder Magic based shield which he uses to both block and attack with. Sora attacks by bashing close-range enemies, while throwing the shield at long-distance ones. Finisher: Sora’s Keyblade transforms into a flying chariot complete with its own blonde Pegasus. In this phase, the player can move around and strike nearby foes with Thunder upon a press of the Attack button.
Form Actions
Shield Spin: Spread your arms and spin at foes. (Combo finisher)
Lightning Circle: Raise your shield and strike the area with lightning. (Combo finisher)
Wrathful Fist: After charging your shield with Shield Charge, select Attack to hammer foes with a giant fist and send them flying.
Wrathful Flurry: After fully charging your shield with Shield Charge, select Attack to hammer foes repeatedly with a giant fist and send them flying.
Shield Charge: Hold [SQUARE] to temporarily guard against attacks from all directions. You can move while guarding, and successful blocks will charge the shield in stages.
Ultimate Finish: Unlock your Keyblade’s full potential and perform a powerful move.
Hunny Spout: [FIRST METER] Hunny Blasters
Description: Sora’s Hunny Spout Keyblade splits and becomes a pair of auto-fire guns; these guns fire honey dippers that are followed by bees, which automatically lock on to nearby targets. Offensive Magic in this form transforms into 4 individual small projectiles that spread out before homing in on the target.
Form Actions
Sliding Shot: Tilt [LEFT STICK] and press [CIRCLE] to fire a blast of projectiles as you evade in that direction. Only works while in combat.
Burst Shot: Fire a volley of simultaneous projectiles (Combo finisher)
Auto-Shot: Hold [R2] to continually fire projectiles.
Multilock: Lock on to multiple targets at once.
Hunny Shot: Launch projectiles that honey coat enemies they touch.
[SECOND METER] Hunny Launcher
Description: The Hunny Blaster guns combine and transform into a bazooka-like weapon. While in this stage, Sora swings the bazooka with slow, heavy attacks when at close range, and will fire large, single shots with strong homing capabilities when at a distance. Sora’s Dodge Roll becomes a Quick Run-like dash. Offensive Magic in this form transforms into single projectile blasts that explode upon impact, releasing their individual element.
Finisher: Launches a large bee made of honey from the tip of the launcher forward, before triggering an explosion that rains honeybees and honey down on impact.

Form Actions
Rising Blast: Knock enemies away and then blast them with a projectile (Combo finisher)
Final Impact: Fire a projectile that results in a wide area blast.
Charged Shot: Hold [R2] to charge up energy and release it as a projectile.

Classic Tone: [FIRST METER] Boom Hammer
Description: Sora’s Classic Tone Keyblade transforms into a giant comical looking hammer that unleashes slow but devastating attacks that can potentially crack the ground. It is effective against groups of enemies, having wide-reaching, area of effect attacks. With every strike, the hammer creates cartoonish-like visuals and sound effects. Sora’s Dodge Roll becomes a Quick Run-like dash. Offensive Magic in this form transforms into close-ranged radial bursts that damage surrounding enemies.
Form Actions
Ground Wallop: Slam a giant hammer into the ground (Combo finisher: while grounded)
Aerial Wallop: Smack downward with a giant hammer. (Combo finisher: while airborne)
Flying Clobber: While in flowmotion, press [EX] to take a swing at foes. (Flowmotion attack)
Guardbreaker: Prevent your weapon attacks from being deflected
[SECOND METER] Clock Drill
Description: The Boom Hammer transforms into a comical-looking drill-like gauntlet. Sora utilizes the drill to devastate singular enemies with heavy thrusts and swipes that break through physical defence, and at the same time deal damage to groups via slamming into the ground, with one of his attacks sending drills to come up in a large radius around him. Like the Boom Hammer, with every action, the gauntlet exhibits both cartoonish-like visuals and sound effects. Offensive Magic in this form transforms into single projectile blasts that explode upon impact, releasing their individual element. Finisher: The scenery around Sora becomes like a black and white film while he launches himself onto an old-timey clock gear on the ground; Sora then proceeds to spin counter-clockwise, turning the centre dial, which then deals damage to all surrounding enemies.

Dire Drill: Lunge toward foes with a giant drill (Combo finisher: while airborne)
Corkscrew Uppercut: Launch foes into the air with a giant drill (Combo finisher: while grounded)
Drill Pillars: When surrounded, call forth countless drills from beneath the enemy’s feet (Combo finisher: while grounded)
Diving Dodge: Tilt [Left Stick] and press [SQUARE] to dive beneath the ground and evade in that direction. Only works while in combat.
Ultimate Finish: Unlock your Keyblade’s full potential and perform a powerful move.
Guardbreaker: Prevent your weapon attacks from being deflected

Element Form
Keyblade(s): Shooting Star / Ever After
Description:Sora's clothes become a dark blue with star patterns. Element Form's weapons are magic-based, focusing on unleashing ranged and/or magical attacks.
Abilities
High Jump: Leap high into the air. The longer you hold [CIRCLE], the higher the jump.
Doubleflight: Press [CIRCLE] when in midair to jump a second time.
Combo Master: Combo continues even if your swings miss the enemy
Leaf Bracer: Always finish casting recovery magic, even if you are hit by attacks in the process.
Magic Combo Thrift: Reduce MP cost by 1 with each successive magic cast.
Magic Galvanizer: Increase the damage you deal with magic in proportion to the length of your combo. Stack the ability to increase the effect.
Magic Treasure Magnet: Automatically draw in and collect prizes from enemies defeated with magic, no matter how far away they are.
MP Hastega: Recharge MP 30% faster when you run out. Stack the ability to increase the effect.
Hidden Potential: Gain additional Strength and Magic when battling powerful foes.
Shooting Star: [FIRST METER] Double Arrowguns
Description: Sora’s Shooting Star Keyblade splits and becomes a pair of auto-fire Bowguns that shoot arrows of light. Offensive Magic in this form transforms into 4 individual small projectiles that spread out before homing in on the target.

Ever After: Mirage Staff
Description: Transforms into a long staff. While in this form, Sora glides alone the ground twirling the staff. When attacking, Sora unleashes Surge-like close-ranged light spells. Finisher: Sora stabs the staff into the ground, causing a tall tower to rise. Raising the weapon up alongside two light clones, Sora then generates a Trinity Limit-esque orb of light that fires lasers down on enemies below in random patterns, as well as lasers that circles around.
Form Actions
Golden Rays: Blast enemies with a criss-cross of magical light. (Combo finisher)
Revolving Rays: Fire blasts of magical light as you turn. (Combo finisher)
Avatar Barrage: Strike at foes along with the doubles you have created with Avatar Shift.

[SECOND METER] Drill Punch
Description: The Hyper Hammer transforms into a drill-like gauntlet. Sora utilizes the drill to devastate singular enemies with heavy thrusts and swipes that break through physical defence, and at the same time deal damage to groups via slamming into the ground, with one of his attacks sending drills to come up in a large radius around him. Offensive Magic in this form transforms into single projectile blasts that explode upon impact, releasing their individual element. Finisher: Sora grabs an opponent with the drill, having opened it up to become a vice-like claw similar to that of a claw game's and extending a line from the gauntlet in a whip. He then begins using them as a wrecking ball, slamming them all over the battlefield to hit surrounding enemies before throwing them away.

Description:The Agile Claws transform into dual yo-yo like weapons. They attack enemies from a distance with multi-hitting strikes, generally excelling at mid-ranged attacks while maintaining heightened speed. Offensive Magic in this form transforms into 4 individual small projectiles that spread out before homing in on the target. Finisher: Sora catches any enemies in front of him with the yo-yos, having turned them into sharp concave claws with eyes that are massive in size. He then begins crushing and mashing all enemies between the two, dealing massive damage and keeping larger targets caught in place while other enemy types get sent flying with each clamp.

Offensive Magic in this form transforms into close-ranged radial bursts that damage surrounding enemies. Finisher: Sora sticks the flag into the ground and generates a pool of water around him that then spawns multiple kraken-like tentacles also made of water, lashing out and hitting all surrounding enemies.

Description: Blizzard Claws transform into icicle-like weapons attached to Sora’s wrists and feet. When attacking, he unleashes fast-paced Blizzard-based melee attacks. Sora’s Dodge Roll becomes a Quick Run-like dash with a swiping motion. Offensive Magic in this form transforms into close-ranged radial bursts that damage surrounding enemies. Finisher: Sora stomps the ground and creates a large snowflake-shaped patch of ice. A tall ice structure begins erupting from that patch as Sora dances and ascends it, before striking it from the top and causing it to collapse and damage surrounding enemies.

Nano Gear: Nano Arms
Description: Sora’s Nano Gear Keyblade transforms into a larger, broadsword-type weapon with the “blade” composed of microbot-like nano-machines. When attacking, Sora retains many of his normal combo attacks. However, the nano-machines in his weapon allow him to adopt combo attacks and finishers copied from the transformations of other Keyblades, such as Twin Yo-Yos' mid-ranged forward-moving attack, Drill Punch's diving aerial attack, Hyper Hammer's air-to-ground slam, etc. This is achieved with the nanos transforming his blade into smaller versions of those respective forms. Sora’s Dodge Roll becomes a somersault. Finisher: Sora generate a portal above him, before jumping into it and creating a sphere of portals composed of the nano-machines. From then the nanos form spikes jutting outwards from the portals in all directions to damage surrounding enemies.

Description:The Ultima Weapon transforms into a rainbow bladed sword. Sora gains 3 crown insignias on each sleeve, stripes on the leggings and the outfit itself turns mostly white. While in battle, Sora wields multiple swords in battle, which overwhelm enemies with fast and powerful swipes which possess great range while rending the ground with each attack. Finisher: Sora jumps into the air and creates a flurry of blades around him, dealing rapid damage before ending with sending a last circle of swords out around him in a great radius.
Abilities
Teleport: Instantly close the distance between you and the target.
Combo Master: Combo continues even if your swings miss the enemy
Leaf Bracer: Always finish casting recovery magic, even if you are hit by attacks in the process.
Second Chance: Always retain at least 1 HP if your are hit while you have 2 HP or more.
